It outlines a methodology to use to gather information and with the assistance of the therapist to develop an intervention plan and assess effectiveness. This form can also be used by therapists as a way of gathering data when asked to provide consultation services on a student/client. This form can also be used in a clinic, or home, by parents, professionals and paraprofessionals.
